Our Endoscopic Drainage Stents are packed with an implant card that should be completed by the healthcare institution/provider and handed over to the patient. The card comes with one side preprinted with the manufacturer and stent traceability information. The other side must be completed according to the example shown below:

The physician or healthcare institution/provider shall complete the card with the relevant information shown above and then give the completed card back to the patient. The patient shall keep this card safe and protected during the period the device will be implanted.

Patient name or Patient ID
Date of implantation
Name and address of implanting healthcare institution/provider
Information website for patients
Devine Name
Unique device identification
